Please complete the applicable intake consent packet(s) and Court and Legal Fees form to better prepare for our first meeting. Completed consent forms will be needed for each individual participating in counseling.

If a minor will be participating, please complete the Teen Intake Packet. Participating adults will need to complete the Adult Intake Packet

Please bring all completed forms to session with you!

*If your child no longer resides with both biological parents due to a divorce or change in guardianship, please bring the legal paperwork regarding custody and guardianship information relating to who is able to seek medical/psychological attention. Custody/guardianship paperwork is required before a minor can be seen in a counseling session.
